Most of the time, it’s because the code expired (it changes every 30 seconds) or your phone’s time isn’t synced properly with the server.
Just make sure your phone is on automatic date & time, then open your authenticator app again and use the latest code. That usually fixes it.
If it still doesn’t work, the 2FA setup might need to be rechecked or reset.
